ATTENTION: New patient Enrollment Procedures

Starting the week of May 23rd, 2022, patients wishing to enroll must make an appointment via telephone at (608) 756-4638 or in person. Enrollment appointments will take place on Thursday afternoons from 1:00pm to 4:00pm.

About HealthNet's Medical Clinic

The HealthNet Medical Clinic utilizes a community-based model where volunteer medical professionals provide primary care to patients. The clinic provides medications to patients, vision referrals, specialist referrals, laboratory tests, as well as other resources and services. Appointments are required.

All of our services are dependent upon volunteers and donations from community members and other organizations. Patient donations are encouraged.

No one will be denied access to services due to an inability to pay. There is a discounted/sliding fee scale schedule available based on family size and income.

Are you eligible for medical services?

You may be eligible for medical services at HealthNet of Rock County if you meet the following criteria:

You are a Rock County resident

Your household income is at or below 250% of the Federal Poverty Level (What’s this?)

You are completely uninsured

When Enrolling, Please Supply The Following:

  • Proof of Address: Lease, Utility Bill, or piece of mail with your name and address on it;
  • Photo Identification: State ID or Driver’s License, passport, school ID, etc.;
  • Income verification for entire household: Paystubs for last 30 days, stating payment in cash/personal check (date, employer/business name/address, hours worked per pay period, and wages)*
  • Copy of 2023 Federal taxes: 1040A or Form 4506-T
 
*Please Note: Income must be 250% of the Federal Poverty Guideline or below based on household size.

When coming to your appointment please remember to bring ALL DOCUMENTS with you.

Your enrollment will be delayed if you do not bring all of your documents. We need ALL financial documents of everyone in your household.
