About Our Medical Clinic

Volunteer medical professionals provide primary care to patients. The clinic provides medications to patients, vision referrals, specialist referrals, laboratory tests, as well as other resources and services. Appointments are required.

All of our services are dependent upon volunteers and donations from community members and other organizations. Patient donations are encouraged.

Every New Patient Must Supply The Following:

  • Proof of Address: Lease, Utility Bill, or piece of mail with your name and address on it;
  • Photo Identification: State ID or Driver’s License, passport, school ID, etc.;
  • Income verification for entire household: Paystubs for last 30 days, unemployment verification, social security award letter (upcoming year), etc.;
  • Copy of 2020 Federal taxes: 1040A or Form 4506-T

When coming to your appointment please remember to bring ALL DOCUMENTS with you.

Your enrollment will be delayed if you do not bring all of your documents. We need ALL financial documents of everyone in your household.
