How To Enroll For Dental Services

To schedule an individual appointment for enrollment, call our dental clinic at 608-756-4638.

Your enrollment will occur at our Dental Clinic located at 113 S. Franklin Street, Janesville, WI  53548. No one will be denied access to services due to an inability to pay. There is a discounted/sliding fee scale schedule available based on family size and income. Please see the link below for further information.

How You Can Be a HealthNet Dental Patient & What To Do Next

You MUST be a Rock County resident! Please click or tap on the option below that best matches your situation:
  • If you have ForwardHealth (Badgercare/Medicaid), you do not need to enroll before scheduling an appointment.
  • To schedule an appointment, call the dental clinic at 608-756-4638 and give your ForwardHealth number or name & date of birth over the phone.
  • You must bring to your enrollment appointment:
    • Your photo ID/driver’s license or passport to your appointment
  • Have no private medical OR dental insurance.
  • To qualify, household income is at or below 250% of the federal poverty level.
  • Call 608-756-4638 to schedule an appointment to enroll as a dental patient.
  • You must bring to your enrollment appointment:
    • Proof of address (lease, utility bill, or piece of mail with your name and address on it)
    • BadgerCare (Medicaid) Denial Letter: Dated within the past 6 months.
    • Photo ID/driver’s license, passport or school ID
    • Household income information: paystubs for last 30 days, unemployment verification, or social security award letter (upcoming year)
    • Copy of 2022 Federal taxes: 1040A or form 4506-T
  • To qualify, your household income is at or below 250% of the federal poverty level.
  • Call 608-756-4638 to schedule an appointment to enroll as a dental patient.
  • You must bring to your enrollment appointment:
    • Proof of Address (lease, utility bill, or piece of mail with your name and address on it)
    • Photo ID/driver’s license, passport or school ID.
    • Household income information: paystubs for last 30 days, unemployment verification, or Social Security award letter (upcoming year)
    • Copy of 2022 Federal taxes: 1040A or form 4506-T
  • If your VA insurance ONLY covers medical and not dental, you may be eligible as a dental patient.
  • To qualify, your household income is at or below 250% of the federal poverty level.
  • Call 608-756-4638 to schedule an appointment to enroll as a dental patient.
  • You must bring to your enrollment appointment:
    • Proof of Address (lease, utility bill, or piece of mail with your name and address on it).
    • Photo ID/driver’s license, passport or school ID.
    • Household income information: paystubs for last 30 days, unemployment verification, or Social Security award letter (upcoming year).
    • Copy of 2022 Federal taxes: 1040A or form 4506-T.
  • We can only serve patients with Medicare Part A and no supplemental plan (Medicare Part B, C, or D do NOT qualify. This is because there is no dental coverage under Part A, but if there is BadgerCare coverage as a secondary, billing goes to BadgerCare).

  • You must bring to your enrollment appointment:
    • Proof of Address (lease, utility bill, or piece of mail with your name and address on it).
    • BadgerCare (Medicaid) Denial Letter: Dated within the past 6 months.

When coming to your appointment please remember to bring ALL DOCUMENTS with you.

Your enrollment appointment will be rescheduled if you do not bring all of your documents. We require the financial documents for spouses, and for parents/guardians if a minor is enrolling as a patient.

Your eligibility for the Sliding Fee Scale is based on your household income and household size and is calculated using federal guidelines. You can find our Sliding Fee Scale here in English and Spanish
