Behavioral Health services for low-income Rock County residents

HealthNet’s newly-opened Behavioral Health Clinic provides comprehensive mental and behavioral health services for residents of Rock County who either are uninsured or who possess BadgerCare (Medicaid). At HealthNet of Rock County, we deliver high quality mental and behavioral health services at affordable prices. 

No one will be denied access to services due to an inability to pay. There is a discounted/sliding fee scale schedule available based on family size and income.

Your eligibility for the Sliding Fee Scale is based on your household income and household size and is calculated using federal guidelines. You can find our Sliding Fee Scale here in English and Spanish.

Are you eligible for Behavioral Health services?

You may be eligible for Behavioral Health services at HealthNet of Rock County if you meet the following criteria:

You are a Rock County resident

Your household income is at or below 250% of the Federal Poverty Level (What’s this?)

You are completely uninsured or you have BadgerCare (Medicaid)

How to enroll in behavioral health services

Please click on the following option that best fits your current position

If you have a ForwardHealth card (BadgerCare/Medicaid), you do not need to come to HealthNet’s offices to enroll for behavioral health services. Follow the steps below:

  1. Call our behavioral health clinic at (608) 756-4638 ext 400.
  2. When prompted, give your date of birth and either your name or ForwardHealth number

What to bring to your first appointment:

  1. Any photo ID (Driver’s license, passport, government issued ID)

Please arrive to your appointment at your specified date and time. Please call our behavioral health clinic at (608) 756-4638 ext 400 if you have questions or concerns about your care at HealthNet of Rock County. 

If you do not possess private insurance, BadgerCare/Medicaid, or any other form of behavioral health coverage, you will need to come to our behavioral health clinic at 113 S. Franklin Street, Janesville, WI, 53548 to enroll. 

You must present the following documents to enroll: 

  1. Proof of Address (lease, utility bill, piece of mail with your name and address)
  2. Photo ID (Driver’s license, passport, school ID, etc.)
  3. Household Income Information* (paystubs from the last 30 days, unemployment verification, social security award letter)
  4. Copy of 2021 Federal Taxes (Form 1040 or Form 4506-T)
